We Need Your Feedback – Survey for Teens and Children

Cockburn Libraries want your feedback, we are looking at all of our Children’s and Young People’s programming and want to know what you think of what we do, what we could do better, and what you would like to see us change including days, times, and events.

If you have a children aged 0-18, please select the survey for the correct age and let us know what you’re thinking. 

If you have children in multiple ages, we’d love it if you could fill out more than one survey. 

All survey entries can go into the draw for a $100 Booktopia voucher, to enter simply include your card number in the last question. T&Cs below.

Survey runs 20 November to 10 December 2023.
Survey can be filled out online or in person at the library.

• The survey is open to all, however the winner must be a member of Cockburn Libraries.
• The winner will be selected by secret prize draw.
• Winners under 16 years of age must have the permission of/be accompanied by their parent/carer/guardian.
• Promoters’ decision is final, and no negotiation will be entered into at any time throughout this survey.
• Any questions regarding this survey can be made by contacting the Young People’s Services Coordinator on 9411 3840.
• Surveys must be received by Cockburn Libraries’ staff by Sunday 10 December at 5pm.
• Prize will be drawn on Tuesday 12 December 2023 at 12pm. 
• The winner will be notified by phone. It is the winner’s responsibility to collect their prize.
• The prize is a $100 Booktopia voucher.
• Should the winner not be able to be contacted after two weeks from the draw, a new winner will be selected.
• Cockburn Libraries reserves the right to defer, cancel, or disqualify any entrant, winner, or attendee of the competition as it sees fit and without reason or notice.
• Employees of the City of Cockburn and immediate families are ineligible to enter.
• The prizes must be taken as supplied and cannot be exchanged or redeemed for cash. Any ancillary costs associated with redeeming the prizes shall be the sole responsibility of the prize winner(s).
